Who should be referred:
- Adults and children with physical
and/or neurological disabilities that prevent them from
doing routine activities of daily living.
- Frail
elderly individuals or couples.
- Young
adults with chronic medical problems that impede normal
daily activity.
All applicants must be eligible for
Medical Assistance. Contact the Department
of Human Resources if you think you are eligible for Medical
Assistance but do not have an MA card.
